I have relatively small feet. I wear size 8 shoes, and I often drive with low sole and narrow profile shoes (Vans Classics or Puma Vedano / "Motorsport" style shoes).
I "toe" with the ball of my foot under my big toe, and I "heel" either with the lower part of the ball of my pinky toe and/or my foot arch.

That being said, the only pedal mod that I feel is needed is a pedal overlay. I use the Cusco Sports Accel Pedal. If you are applying sufficient braking and have good foot position, then the pedal overlay should be sufficient.
If you add the spacer to the mix (which raises the position of the accelerator pedal), it will likely be too high for comfortable heel-toe shifting when under hard braking. In other words, the side of your foot might collide with the side of the accel pedal.



Finally, a few other considerations you might want to be mindful of to improve your heel-toe shifting:
  1. Practice, practice, practice. If you have the right technique, your heel-toe method will be consistent, whether on "track" or "street"
  2. The stock throttle map is a bit mushy at the beginning of the pedal throw. That means you really need to emphasize the accelerator blip and give it gas to get the revs you need. I have a "race throttle" map from my Stage 2 tune, which was night and day for heel-toe.
  3. The higher the revs, usually the easier it is to blip the throttle. Generally, I don't do heel-toe unless the engine revs are at least 3k RPM. Lower than that, there is usually not enough time to comfortably rev and shift through the gears.

I've had the SRP pedal for about 5 years now. It's been a blessing since the day I've installed it, makes heel-toe so much easier. Much cheaper than the Velox spacer too.

You can get a 10mm(3/8) spacer under the stock pedal assembly and still use the stock hardware to mount it. Keep in mind you will need to adjust or space out your pedal stop too. I cut a piece of UHMW and used it as a spacer with the Cusco pedal. Works perfect for me, I wear 8.5-9s. I am working on some 3D printed spacers now out of PETG with different offsets.
