While not tightened and fallen off seems most probable and while by specs it should be more then strong to give up first there IS chance for some bolt once in a while have manufacturing defect, even for OEM parts, even for japanese. I doubt each and every oem bolt is always carefully x-rayed/checked for any hidden defects. They are too cheap and too mass produced for that.

You have a point. I'd almost suspect sabotage over that though...

I mean this is my second twin with coils and a supercharger. Pretty mechanically inclined. Just glad I caught it before i began driving it with the supercharger and really fucked something up. Last time it was touched was when I went in for an alignment.

do you know if you drove the car at all like that? I would assume you would notice it pretty quickly if you were missing one of the bolts when driving on the street. good thing you caught it though

I did notice it. But I didn't have time to fix it. So I still drove on it. Didn't realize it was missing a whole bolt. Got time to look at it today.
