I'm not new to stick shift cars but kind of new to heel/toe, the thing I didn't get until recently is you need to lift up your whole foot, use the ball of your foot on the brake then kind of angle out your heel and roll over onto the accel pedal. At least that's what's worked for me.

In my experience there are two main ways to heel and toe:

1. Left edge of the shoe on the right side of the brake pedal. Roll the foot to the right to catch the gas pedal with the right edge of the shoe.
2. The ball of the foot on the brake pedal, shoe angled diagonally between the gas and brake pedal. Catch the gas pedal with the bottom right of the shoe.

I find that it greatly depends on the car, shoe size and shoe dimensions as well as personal preference and kinematics (position of the seat and steering wheel, proportions of the leg, etc.).
I used method 1 in my Focus ST, but switched to method 2 in my BRZ.

That's how I do it. I was never able to keep consistent pressure on the brake pedal doing it the other way.

It was really difficult on a sportbike before they started using slipper clutches. My bike years ago did not have one. Don't get it right and you lock up the rear going into a corner.

I installed my SRP pedal cover. I have been using their wider gas pedals in most of my cars.

On my first cars, heel and toe was more double-clutch shifting. Some of my cars did not have a synchro first gear and the other gears had synchronizers that were total crap and would only work if you took your time between gears or were good at heel and toe. My later cars had full synchro gearboxes with floating synchro rings and not the press-on-the gear hub type. We affectionately call our transmissions "Crash Boxes". One time I missed a shift from second to third on my Triumph and knocked a tooth off the cluster gear. I still had a first gear if I needed it, but it went thunk, thunk, thunk. Those of you that were born after the first Moon Landing won't know of those joys.

my feet aren't big enough for #1, I'm stuck with #2 as my only option and even then I have to lift my foot up and angle it almost 45 degrees to reach the accel pedal...but I'm a women's size 8.5

I'm on superstreet-1 coilovers, lowered about 1.5" (last I checked, it may have settled a smidge more) and about -2.5⁰ camber all around. The wheels are 17x9" apex arc 8 with 245/40R/17 sumitomo HTR PS-03 tires.

Do you think the rears would clear with a bit less camber? -1.7 or so?

Probably so, I ran the wheels/tires for a couple weeks with some 3mm spacers up front on stock suspension and didn't have any rubbing. It pokes a little bit out the top without negative camber.
