Problem with TRD quickshifter gettingloose
This is the second time this bolt (pictured below) has came loose on me after installing the quick shifter. Both times I re-applied loctite to the bolt and screwed it in. The first time I noticed a black washer of some sorts that shredded off. I'm not sure if that is a big deal.
How can I make sure this doesn't happen again?

I've been having this issue as well. The last time I retightened the bolt I actually took some of the loctite off and it seems to have helped a bit.

Use a Nylock nut, it has Nylon inside the nut and works well. Use loctite 272 if you want high strength

This happened to me when I first installed my kit. I didn't have the bolt threaded properly in the adapter piece that comes with the kit. It was surprisingly difficult to seat properly but it finally took and has been holding strong for several weeks now. Coat that thing in loctite and make sure it's threaded properly.

I'm talking about the metal piece the bolt threads in to. Only a small amount of the bolt's threads actually engage so it is really important that you get it seated properly.

Lock washer, and make sure you're torquing it to spec or maybe a few ft/lb more.

Blue locktite is giving in...will use red in one week...see how much it holds...if not, i'm sealing that shit forever with something...
