Posted this picture on another forum last night: Immediate response was "Your going to have much bigger problems if you drop those bolts down intake holes, I would advise some paper towels in intake." So you shove paper towels (or rags) down all the holes, forget to pull them out and boom, something not fun happens. |

If it can happen in an OR with multiple sets of eyes on the patient, it can definitely happen in a garage with one set of eyes on the project. EDIT: Ugm what @Tcoat said....

According to the Center for Auto Safety, a recall issued last fall by Toyota Motor Corp. and Subaru Motor Corp. to address a valve spring issue that caused the engine to stall may not have cleared the cars back to 100-per-cent health. Tales of stalling and engine knocking are still allegedly being reported. The automakers recalled 411,000 vehicles globally, including 140,249 from Subaru and 80,000 from Toyota in the U.S. Canadian numbers haven’t been made available for Toyota, but 19,164 Subarus were recalled nationally. The Washington D.C.-based non-profit consumer advocacy group told Automotive News it believes the repair itself is busted and suggests the NHTSA (National Highway Traffic Safety Administration) might want to take a look-see for themselves. |

I had to get a non magnetic bolt out of a catalyst once, about 18" down around a bend. That is the only hole I shove a rag down to this day. OM642 combination box with the top elbow removed. That was a fun one |

It was probably covered up by a Maine Coon cat, and the mechanic didn't notice the shop rag under the cat when he lifted it off the cover. Could happen.
