Personally, I have trust issues. So I've never used them.
I mocked up my own password management system once, during a period when I had to keep track of dozens of passwords for client and partner systems.
It was rather cumbersome, but it was heavily encrypted and masked as junk files.

We have to use a commercially available enterprise solution at work and requires a RSA token to get into the vault.

Having said that, the other night we needed to retrieve a password to run a crucial job at a particular time during our shift. A colleague wanted to make sure the logon works an hour before, and found that the specific password field in the vault was null!!

Whoever updated the password last didn't do it properly. We had to get a hold of that person to beat the password out of her.

Agreed. I use the popular open one figuring it has enough eyes studying the code. Even then, look how long OpenSSL was out there answering malformed queries.
