1) it was their fault, due to negligence, carelessness, etc. 2) it was someone else's fault, or outside of their control. when #2 happens it's often said it's due to bad luck. because if you had good luck, you would have maybe avoided the accident. Even though your skill/care had nothing to do with it. So which was it? I'd say #2.
