I wouldn't doubt the lean, overly rich had been an issue throughout the tuning process. The computer tried to compensate though i believe, late in the game though, after tear down, the most heavily damaged piston, had completely melted down, its spark plug was no longer three pronged, but nubbed, and it was speckled with aluminum.

I believe, based on my amateur analysis, it detonated, damaging the spark plug, which broke down, creating a hotspot, which settled on the piston, and continued to detonate, creating heat, thus melting through the piston.

I have gotten a rich misfire, can you expand on that? Is it just too much fuel the spark plug can't ignite? (Honest question).

Sure. There are only certain air:fuel ratios which will support combustion. If you go beyond this, either way, you get a 'misfire'.
