Like say your doing sixty coming up on a stop and you throw it in first with the clutch in without having slowed down much. My sister did that when she was first learning to drive, the clutch just exploded into tons of dust.
Note: the clutch doesn't always explode immediately, it can just be weakened enough that after a while it falls apart under normal driving.
