Timing as has been said. Don't let off the gas completely until you disengage the clutch. Practice not touching the clutch, just letting off the gas until the car is at a steady speed, not accelerating or decelerating. That's the point at which you press the clutch.
Many modern cars you can just let right off the gas pedal and they'll not fully close the throttle straight away so you won't get the jerk. Thankfully Subaru/Toyota decided to build a car that actually does what you tell it. They are a dying breed.
