Shouldn't be very hard. Just position your hand (use the pinky) so you can lift the lock out and put your hand sideways on the extender.

OEM knob is a lot heavier.
You can feel the difference in how light and jittery it is compared to stock.
Definitely couldn't use it on a regular basis. Looks badass though.

For those with the Torque Solutions collar, does it raise the overall height of the shift knob ? And if so, is it by a noticeable amount?

It does but not by very much, maybe 1/8-1/4". not very noticeable.
