This was over a 28 period not just yesterday. Two of my 4 plants are in the top ten safest of the company's 400 locations (world wide). The other two are in the top 50. Not that we don't still have issues they are just controlled and kept minor. If I speak that something "exploded" I actually mean it exploded and it isn't just a figure of speech. Mind you are would not come onto a forum and say that we had an explosion or I would be in big trouble if anybody found out.

It was urban legend that the now defunct Oakley Sunglasses plant in Nevada had a furnace exploded underground due to the magnesium involved in their titanium alloy manufacturing.

I always wondered if it was marketing BS.

None of the incidents were with my current employer.

Let's see what I can remember from others.
Arm ripped off by conveyor. Didn't lock out line before working on it.
Electrocuted fell and struck head. Died of head injury not the shock. Unplugged extension on a plasma cutter instead of turning it off at box. Wire inside cord broke off.
Foot ran over by forklift steel toe twisted and cut off whole end of foot. Standing talking to driver and did not step back when they drove away.
Fell (jumped?) off roof died of broken neck. We never knew what they were doing on the roof since not their job. Suspect suicide but was never determined.
Two hands in 40 ton press. Press cycled. Had bypassed safety switch.

I could list the stuff from my very brief stint as an army ambulance driver but then things get really gross!

Today is better. Inventory is nearly done, just waiting on a coworker to get here, but he has a 2.5ish hour drive ahead of him, so we'll see what time it is when he shows. Can get all of his equipment in, then the list should be down to only 98 items outstanding.

I'm throwing in the towel early today, and leaving around lunch time and will leave filling out forms and boxing up equipment to get sent for calibration tomorrow. This way, I can sit at the gf's and play skyrim until she gets home and complain about how hungry I am to her while she's in class.

Inventory is nearly done, just waiting on a coworker to get here, but he has a 2.5ish hour drive ahead of him, so we'll see what time it is when he shows.
