I immediately answered, then retracted because I realized that, while I'm familiar with the behavior - and yes, it finds an icon on next visit - I can't explain why some made it across the upgrade.

Here's my theory. There's a database, and there are cached image files. Deleting cache doesn't clear the icons for a current installation but maybe the image files are used on upgrade or migration.

It's interesting but not enough to overcome my laziness.

Yeah I had the same guess about a cache. Inevitably they'll show up at some point.

Ok, here's a solution.
If the address is "http........" the icon won't load. If the address is "https...." the icon will load. I thought it might have something to do with having HTTPS Everywhere installed but disabling that had no effect. I had to alter the address myself.

This could be tedious.


Update.
This doesn't work for all sites and some sites that already have https don't load the icon.

Ha! I looked a bit deeper because I have a bunch of bookmarks that I haven't visited in a very long time. Found one that was unencrypted. (http) and observed the same as you.

Find another unencrypted link. I bet it redirects to an encrypted one.

Now notice that your bookmark indicator (the little star at the right of the URL bar) is blank/white. The redirected URL doesn't match so it's considered a new website.

I manually edited the target of that same old bookmark and used it again. This time, the bookmark icon (star) turned blue and the bookmark icon loaded.
