My brother and his wife are finally getting vaccinated. I think the Delta variant is making some people reconsider their position, but unfortunately it may not be swaying enough people. Our mandates are hospital and death driven. Deaths are rising slower than cases, but hospitalizations are rising at a point where capacity is low.

Our volume has gone up to pre-pandemic levels with 10-15 codes per shift and 135 patients a day for a 26-bed, level 2 trauma ER. Unfortunately, acuity is high and the new COVID surge just adds difficulties. The hospital is routinely full with 10+ admits holding for hours to days in the ER waiting for rooms to open in the hospital. During much of the waves from early last year, the ER dropped to 65-85 patients per day, which felt busy with high acuity because of all the donning and doffing of PPE and because of the resources needed to care for the COVID patients. This could be worse for hospitals because the volume is normal, but we also are adding the new surge and all the people delaying treatment for chronic diseases finally coming in. Our waiting room times are far lower than the national average, but have increased from 45 minutes to 3-6 hours, and from 0-6 patients in the waiting room to 15-25. We saw a day last week with 32 patients and an 8 hour wait time. We were admitting and transferring and sending people with appendicitis and cholecystitis directly to the OR from the waiting room. We went on divert for several hours a few days ago too, which is practically unheard of for our hospital. I think it may have happened three or four times in ten years with only twice being because of volume. The other two was because we had an active shooter in the parking lot, and the other time was because of a guy with a machete in the waiting room, so temporary diverts. When we had 240 patients a day during the 2019 fires we didn’t go on divert. When we had 165 patients in one day with 34 admits in the ER and were running codes in the halls next to hall patients that were admits, we didn’t go on divert.

Government officials won’t just look at cases or even deaths, as much as, hospital capacity and ICU beds, so even with public resistance and resentment, we could see new restrictions and mask mandates going back in effect.

As an aside, I was in a team meeting this morning where we were reviewing the boarding (admissions) process as well as discharge. There are different codes for the types of Discharge, one being Death.

In 2020, we had the same number of discharges in one of the early months of Covid from Death as we did in all twelve months of 2019. That's a sobering statistic.

This next wave that appears to be happening according to that graph should be known as the "I told you so" wave. The one where we feel less sympathetic toward many of those affected being aware they knowingly brought this onto themselves and very possibly others.

Assuming the data is somewhat accurate, knowing this, what can you do? The obvious move to recruit strong influencers turning this into a racialized issue? And who are the broadest influencers among blacks or Hispanics?

That's the perplexing part. KFF donations are 95% from Democrats who claim to have minorities best interests as part of their platform. KFF may have an agenda besides reporting their findings as fact. Possibly they're thinking that this data will make it into media viewed by their target audience and motivate them to get vaccinated. Any suggestions of racism against KFF don't make sense.

I've read your post a few times and still don't understand, but I live in the Center of the Known Universe, Oregon, where the further away you go, the more things tend to speed up.
