No. I think it said it came out of the back bumper of a Saturn.

If op doesnt have that knowledge, then he/she should go to shop

On the other hand, it pads is gone and backing plate is hitting rotor, you would get some vibration, metal to metal grinding noise, and reduced brake efficiency. And if rotor is under spec, you run into danger of brake pad dropping out from caliper, then you would loose brake completely

Op doesn’t seem to experience any of that so I feel at least some pad materials are still present.

Again, if one doesn’t know how to identify how much pad is left, one really should take it to the shop. It’s for the safety, of op and of others on road

Probably, guessing from the description, there is a little pad left but the wear indicators are touching the rotors. That is why it only appears with certain brake pressures.

Even if it isn't the problem checking pad thickness should be the very first thing done with these symptoms. Assuming they are fine and trying to fix it through other means or looking for other issues is just pissing into the wind.

Who runs "several thousand miles" with a known problem? Good news is you can get a good deal on pads and rotors together.

my clutch squeaked for a good 10-15k miles.

knew it needed a fluid change, just never got around to it.

i've got a whiteboard by my back door with all the things that should be done, can be done, and i want to do. priority is given to which part breaks first.

Questionable brakes would be at the top of my list.
