The fix is really very easy. I've explained it to Barloworld Centurion so they should be able to help.

Yes! This fixed the 4000RPM+ rattle/vibration!

-No tools needed
-Jack up the car so you can reach around underneath comfortably.
-Pull the shift linkage rubber boot carefully away from the shifter and slide it onto the gear selector shaft; lots of stretching needed but it can take it!
-Do whatever is needed to keep the washer from rattling. I just applied some thick silicone grease, and oriented the washer so the convex side was on the plastic bushing and concave side on the spring washer.

Maybe this issue has to do with how this conical washer is oriented?

For anyone interested in getting shims or alternative washers, I took some pictures and measurements while I was there. Last picture is linkage pin diameter than the washer fits over.
