I hate to say, I've been told many time over the years that there is not a single bread of dog that will shit/piss where they sleep on purpose. Once they do, for whatever reason, you lost the battle. Wish I had some words of wisdom to help. My advice spike should be to get professional help.
Edit: sorry , I just read your long post where he hasn't gone in his crate. Good news. Keep taking him out more, always say "potty" or whatever word you want when he does go outside, so he learns on command " now is the time". Keep working at it. It will get better. It was on a cheap rug carpet from Target that I use to wipe my feet on when I come in from the balcony, so I'm not even that worried about that. Easy to replace. Better that than the actual carpet on the floor.

I think there's a major difference between sharing a (assuming)5'x5' section of grass with a dozen-plus other dogs, and running through a 20 acre park of grass with him.
For that, I can understand your vets apprehension to too much other dog bathroom activity in a small area. Which is why I suggest the park. Lots less dog bathroom activity per square foot, so the vets concern should be negated. And you can never play enough with a puppy! If you figure out how to bottle that boundless energy, I'll take 2 bottles!

Sounds like good bait, wasted....
There is nothing about sushi I like, but that's mainly because I can only eat seafood as the good Lord and my Mother intended, breaded and pan or deep fried, served with hush puppies and coleslaw. There are a couple of exceptions (shrimp I can eat steamed or broiled, but still prefer fried, tuna and shark are good as steaks but not raw/rare, crab steamed, but I can't eat it)
