For those who may still be curious what the issue was. I ended up taking it to a super reputable shop and had them drop the trans. The issue ended up being the clutch. It was installed improperly when it was replaced 15k miles ago and the clutch plates were bent and not making proper contact with the flywheel. I’ll leave some pictures, you can clearly see the inside of the clutch plate looks new and the outside is worn. If you live in the Bay Area I couldn’t recommend CM Autohaus more!

Not more than what the mechanic told me unfortunately. He just said that the clutch disc were likely bent during installation. I asked if it could be caused by bad driving and he said he didn’t think so.

If I had to guess, the input shaft didn't get aligned properly then forced together by tightening the bell housing

Probably not, in that case the input shaft bearing would be damaged. Usually when a clutch disc is installed the wrong way 'round the, hub doesn't clear the flywheel bolts and the clutch can't even release. I can imagine a pressure plate being incorrectly mounted, but why? I have a hard time imagining when the guy could possibly have done so the clutch would wear only on the outside.

But facts remain facts, that clutch disc was obviously not rubbing right.

When the previous clutch work was done, was the flywheel replaced or was the old one resurfaced?

I believe it was resurfaced the last time. Replaced this time.

That's probably where the problem occurred. I've seen several posts where the machining was done incorrectly. Most issues were with vibration.
