Then we would just get random spam posts followed by a PM. I really don't know why a captcha or something isn't a requirement to make an account.

Eh, it's just one example qualifier. Captcha's would definitely be preferable. I just don't know if the version of vBulletin supports that. The problem is that the software itself seems pretty neglected.

So if we can't get something like Captcha to work, we need to work with what features we do have, which would begin with limitations on features.

There must be some basics available with the current version. Starting with post count-limited features, and then maybe even some kind of filter that checks for obvious spam phrases/terms.

I doubt that it is bots responding to the WTB posts. They may alert the person that the posts exists but even as good as they are they still can't go through the process of PMing with exactly the right response.
The only true defense against real live scammers is to be aware of their patterns and be suspicious of ANY reply that seems fishy at all.

there's a "random question" that can be changed to minimize it for a little while.

had the same problem on another forum. spam starts taking over, admin would change the random question to something else, spam would take few weeks, but then become what it was before.

then the forum died for a number of external reasons, and the spam stopped coming around so much.

so spam just means a healthy forum.
