I LOVE these bad boys!
I installed mine today, I took my headers off.
I have two questions.

1. Did I have to put the black metal plate from stock mounts onto the new mounts?
I tried at first but it kept sliding off, so I just took it off.

2. Whats the deal with the two bolts on each mount?
Had no idea how much to torque, so I tighten them until I passed two threads

Yep. Just to confirm: it requires unbolting the header and lifting the engine quite a lot. No other way of doing it

If you bought Perrin mounts, no , no need to transfer the palte from stock mounts.

The bolts on the mounts are adapters to fit what are WRX/STI motor mounts originally to our car.

I didn't use a torque wrench either.

What's the NVH like on these things? I had some mounts on my altima and the vibration was bad (wonderful) enough I had to modulate the throttle so my rear view mirrors stopped shaking! To give some reference points, I've done subframe mount inserts, trans mount insert, diff mount insert, and shifter bushings; I'd call my gear whine 'noticeable but civil.'

I bought a set, I think I'll put em in when doing my header, judging by all these horror stories!

What is the stock durometer anyhow?

Yes the job was so much easier without the header, I also took off my strut bar and intake since I didn't want to smash anything when raising the engine. The NVH is most when turning on the car and taking of from first

The Cusco mounts don't use them either they are constructed completely different than the stock mounts. I swapped the Cusco for the STi mounts, too much NVH.

When did you realized that the rubber part of the mounts melted? Did your car start to shake due to poor motor mounts?

Just replaced my motor mounts. It's not that bad, guys.

For tight bolts: use a longer ratchet. And I was able to get the driver's side mount out with the manifold still on. The trick is, you remove the lower manifold heat shield and then fold the offending flange out of the way. Bend it back to stock and reassemble, zero rattle due to the pretty fantastic heat shield design.
