In select tortured cities, yes. The one in Atlanta is always packed. I own some IKEA furniture I've had for literally 35 years, some of the other didn't last 5 months.

I've never had a piece missing or misdrilled, but also haven't bought anything there in several years. Maybe their QA is slipping.

Or perhaps it has always been hit-or-miss...

The "three chairs" incident happened about 18 years ago!

True story: My wife bought three identical chairs from Ikea... first trip back: missing pieces in chair 1 (I checked the other boxes, they were okay). Second trip back: holes not drilled in some pieces of chair 2 (checked the last chair, it was okay). Third trip back: cushion sewn incorrectly in last chair. Fourth trip back: wife decided she didn't like the way they looked in the room.
Nope. It is missing the activation energy.

Dunno...

I can't read the instructions, perhaps they have a pictogram indicating you should open the tank valves and bash two of the (presumably) steel valves together?

Close off the room, open the valves. Once you hear the hissing stop, light a match. Boom done!
