There's actually a screw where the hole is for the shift knob, you have the remove that first screw then the actual screw-hole will be under. As for getting it lined up straight, I placed it straight before screwing it in, the started turning in/screwing it after lining it up. If that still doesn't work maybe a couple of washers will do the job.

I really wanted this knob until I saw they said it is made of wood and would break during hard shifting.

Haha, that looks awesome. I don't think I would like using it as a shifter but I would totally use that for an e-brake lever.
