During a regular service for my gf's prius they rotated the tires. Later that day I noticed a lug nut sticking out and it was 1 turn from falling out. I checked the other wheels and each wheel had one lug nut 1-2 turns from falling out.

Sigh.. went to the dealership and they put on 7 miles to see a 5th gear shift issue and a rattling. My MPG dropped from 30.0 to 29.1 in 7 miles (over course of 300 miles).. I feel your pain.

The Toyota dealership I go to always records the mileage when I bring it in, even for something small like the alarm install.
