RI Window Tint Exemption

So in RI there is a medical tint law exemption (allows you to legally tint your windows). I've tried google/dmv searches to try to lock down what exactly is needed to get one. I have heard that it is a doc note, I have heard its a form, and I've heard that its a complete submission process that your medical records goes in front of a review board and they decide.

Can anyone clarify the process for me, let me know where I can find said form, or maybe steer me towards a sympathetic ophthalmologist?

Forum note: I'm aware that tints make it more likely to get pulled over, etc, etc. Save your preaching, the 'roll down the windows' comments, and whatever other sandbagging comments you have. I'm just looking for information that can help me obtain the medical waiver.

alright, well if no one knows, does anyone know a reasonably priced tint place in RI. Preferably around W Warwick?

Edit: I called MPs Aftermarket and they quoted me $250 which seems pretty high.

In NJ I got Charcool 35 for sides and back for $240, so I don't think $250 is out of line.

yea, I threw in the 'seems' because I have no idea. I'm sure there's somewhere in RI that can beat that though, I just expected it to be in the 100-150 range

$250 is a fairly standard price for a quality tint. Anything below $200 will typically be cheapass tint that will peel and bubble over time or the install will not be as nice (i.e. edges trimmed unevenly, shortcuts were taken during the application.)

I paid $250 for my tint and it looks flawless.

It is determined by your eye doctor. I have a buddy who got really friendly with one if you know what I mean, and she wrote him a "prescription" that he keeps in his car (Evo8) and if he gets pulled over, all he has to do is flash that thing. Supposedly he has very sensitive eyes.
