That can be just positional. The person looks like they are in a bed and not compliant with raising their head. In fact, they may have not been breathing in and holding their breath, which is why the diaphragm looks high to me. I’ve heard docs say the lungs can look bad just from not holding the breath in. Anecdotal images, but probably true.
