Welp, the CEL trifecta came back even with the fixed header. Found a damaged MAF wire that rubbed against the accessory belt. Fixed the wire and trimmed the alternator cover so that I can run the MAF wire on top. After several drive cycles no CEL so far and hopefully it stays that way.
