Four possible causes for the accident:

1) Driver excess: too fast, too sloppy, poor reactions, removal of safety devices or a combination thereof.

2) Road or weather issues: loss of adhesion due to oil, wet leaves, animal... etc. Sudden changes of weather (tornado, microburst...). Road changes due to fissure, earthquake, bad camber, landslide.

3) Driver inattention: dozing, distraction, involved with passenger.....

4) Vehicle failure: stuck throttle, tire failure, failure in driveline.......

Any of these, alone or together, could have caused the accident....

Interestingly in almost all accident investigations it is found that there is more than one cause (e.g. the distracted driver braked too late on wet leaves).
