All seem like possibilities, but I'm going to go with, your rotors are warped. Happens when you have misaligned suspension or you're really hard on your brakes and one side/rotor get's too much heat/pressure. That warps your rotors and you can do one of 2 things. 1. replace that bad rotor or 2. grind or lap (is that the correct term?) the rotor back to spec. Good luck.
