UPDATE:
As mentioned before, my 811 with a new flash did NOT resolve my issue. Toyota ordered a new ECU for me, and surprisingly, the model number is different.
Older 811 ECU: SU003-04522
New ECU, I just received: SU003-04568
Hopefully this resolves my issue.
