The removal/installation procedure is pretty clear in the manual. I haven't looked at much in a while but I believe you are supposed to index crank and cams on timing marks, remove timing chain from bank one, then reindex to different timing marks and remove bank two timing chain. It sounds like he tried to remove bank 2's timing chain while on the first set of marks and it spun and hit the piston or it had valve on valve contact. I would call Toyota's customer service and explain the situation. It may put pressure on the dealer.
