Like I said, everything north of the Bay Area and Sacramento is 707 and 530, so it covers a huge area, and as you can see by the maps, the population density is fairly sparse. This means that most towns, townships and rural areas are exempt, assuming they are remote enough from a city. For instance, Sebastopol has a smaller population over a larger area than Cloverdale, and yet the residents there aren't exempt because they are 8 miles from Santa Rosa versus 40 miles, respectively. Moreover, Cloverdale has limited locations for getting a smog and the closest DMV is back in Santa Rosa or north to Ukiah. I am assuming this is how they think about it.
