I'm going to post my DM response here so everyone can see it.
No problem, I'll try and explain this as best as I can.
The internal part of the throw-out bearing is plastic. This plastic essentially just glides back and forth across the shaft on the input housing. When the bearing get old, or if there is a clutch fork issue. The bearing will not slide properly on the shaft causing it to bind further degrading the bearing. The shaft itself could be taken out and cleaned however I have had no luck removing all of the worn plastic marring on the shaft when installing a new bearing. If the new bearing isn't installed on a clean shaft then the plastic marring left behind will further degrade and prematurely wear out the new bearing. Since it's a pain to get to it, I look at it as a rather insurance part. It may not need to be changed but I don't think you want to do what the rest of us have done and drop the transmission again or pay a shop to do it the second time.
