Thank you for your detailed reply, the more replies I read, I get the feeling that I'm just stubborn. Until I'm asked or corrected, I'll continue as I have been although outside of the workplace, when I go see a doctor or whatnot, I address them formally regardless of how close we are (unless they specify otherwise).
