That's the hole that should be capped with the rubber grommet during PDI. Water and road debris can get up there and eventually cause rust or rattles but I would personally worry about a small animal getting up there and taking a shit or dying. Ive opened my hood and found poop on top of the engine because an animal found it to be a warm dry safe place to take a nap in the winter.
