To add to the good points already mentioned, I think posture/flexibility for some plays a part in bad shifting. I suffered from it for the longest time, due to poor health and numerous ankle injuries to both ankles over the years, which I neglected. That's vital to being able to modulating the pedals smoothly.

Ever since I went back to the gym, and took physio to bring back strength and flexibility in my ankles and knees my shifting consistency returned immediately.
