Is this a TOB issue?

My clutch feels like it's rough/rubbing against something when I press it. Kind of like a notchiness. This only occurs when I am already moving (in gear)

When I am in neutral it feels smooth as butter. However, at times, in neutral it also feels rough while pressing it.

With the car on, and in neutral, if I press the clutch and release it, I can hear an audible clunk.

Any ideas?
