I had this code triggered when my secondary o2 sensor was pooched. Part of the problem was that the OFT tune I had prior to going FI and ECUTek, disabled the code P2096 along with P0420. The sensor was likely failing long before that but I wasn't getting a CEL so I was fat dumb and happy. With my EcuTek tune only P0420 code is disabled since I am catless. The ECU still needs the the secondary sensor for keeping LTFT in control when in closed loop so disabling it or shutting the sensor off isn't a good practice. A spacer can also trigger P2096 as it is moving the sensor out of the exhaust stream.
