I just had mine fail last Sunday. 84,150km approximately. My car is an August 2012 build.

Throw-out bearing and pressure plate fully covered under 5-year/100k powertrain warranty, although the service manager told me directly he may have to pull some strings and deal with some resistance from Subaru for the claim. I had the car back in my hands by Thursday (Subaru had it for 3 days total, and it was finished on Wednesday, but I couldn't get there to pick it up before they closed that day).

Clutch and other components were not damaged by the bearing failure and I elected to leave them as is for now (55% left on clutch).

Month and year: October, 2016.
Mileage: 84,150km (approx.)
Warranty covered: Yes.
