Transmission or Clutch Taking a dying?

I have 120k miles on my car.
Here's what happens:
I have an Exedy Stage 2 Clutch. About 10k miles in, I started having issues with getting locked out of 1st gear from a launch, but would shift smooth while driving normally and hard driving. Now the more miles I place on the clutch, it is becoming increasingly hard to shift through the gate under full throttle redline shifting. And when I shift sometimes, I'm either locked out of shifting from let's say, 2nd to 3rd gear, or I grind gears or sychros. This never happened earlier.
When I shift under normal driving conditions, the shifting is ok.

Now I'm trying to figure out, are the sychros/gears going bad, or is my clutch not completely deactivating when I push in the clutch. Also, since I installed the clutch, it has a rather low catch point but the grinding and lockouts from 2nd to 3rd, 3rd to 4th etc just started recently.
