Well, usually, when a synchronizer is bad (or going bad) the transmission will "grind" going into gear. After a synchronizer has been eaten up, about the only way to shift gears, without grinding, is by double clutching and/or rev matching or jake shifting.

It's been my experience that when I "grind" going into 5th or 6th, it's because I've "lazy legged" the clutch (didn't push it all the way down).

Yep, slow, firm and steady ......that's what she likes ......
