I've been stuck in the middle of organization charts with those people over, under and to either side of my little rectangle.
But I never thought of it as a culture, I always saw it as fear. A paralysing fear of being responsible for something that goes wrong.

Personally, I've been responsible for lots of things that went wrong. And I have taken charge of fixing most of them.
But once in a while, I have taken responsibility for stuff that has turned out quite OK. And that's a good feeling. A feeling those paralysed people with their frightened little minds never get to have.
