1 Are you running a aftermarket wheel and are they hub centric? Actually measure the hub and wheel, I have seen wheels millimeters off.
2 Are your lug nuts open ended?
3 Do you torque in a star pattern?
4 Are any lug nuts loose or rusted when removed
