1, pretty much any shop would do it. If you have basic tools or space to work at you can do it yourself. Brakes are pretty easy. That's if you have the tools and space to work on.
2. Not really. If there are deep grooves or the pads are worn out highly uneven, or are too thin below spec then yes. As for the fluid, if it's been a few years or you reached the service interval mileage then it's time.
3. Yes. But it might be a good idea to check the ware on them. If you haven't changed them in a few years it might be time. Depends on what you see.
