Pistons are never exposed. Also you have to remember that with the weight of the car on the suspension, under static sag, the shaft will be retracted into the shock body. Although at full droop that portion of the shaft will indeed become exposed if you cut it(like when you take it off sweet jumps and stuff).

Edit: it looks like the boots just slips over the ends of the shock bodies.
