You're already doing well by slowly changing your lifestyle habits. For myself, I've found that dramatic, sudden changes have awesome short term effects, but unless strong-minded, one returns to their old ways. Doing it slowly allows it to become a new lifestyle, rather than a diet.
For working out, again ease into it with realistic shorter-term goals. Personally, I've done the P90x program as well as the Insanity program, and they work well, but after a few rounds, I've found I'd rather buy a Groupon for a group exercise class instead- it gets me out of the house, surrounded by like-minded people and is easier for me to stick to as now I'm accountable- if I don't show up, people ask

Check out some routines others have done- good, real advice here, but you're already off to a good start. Any activity is better than no activity. Today I went for a 1 hour brisk walk full of hills and pushed myself to keep the same pace uphill as downhill.
Another tip that I now live by as it keeps me on track: write everything down, so you can see where you came from. I used to workout just to workout and it was fine, but I had nothing to refer to. Also, take photos along the way, and weigh yourself. After month 1 of P90x, I lost 5lbs. I've never been a large girl, but 5lbs was depressing until I saw the photos and what 5lbs looks like. So take some photos, print out a calendar, take your basic measurements and keep us posted
