That's a great question and one that I get quite often from my patients. I can tell you that they are all great methods of exercise. I believe in each one and so do most of my colleagues. I have not personally performed each program per video instruction wholeheartedly.
P90x theory and principles are great because our bodies are great at adaptation/maintaining equilibrium. So muscle confusion is very important. Basically, don't get stuck doing the same exercises week to week. Same goes with your nutrition. Always change the ways you eat and what you eat. Your body and metabolism adjust quite quickly.
Crossfit and Insanity are along the same lines in terms of methods for improving fitness similar to P90x.
It's all about periodization. Moreover making sure you are always challenging your body through many different methods of training/physical stress (i.e. exercise). Those programs were made based on the basics of strength and conditioning. Interval training, aerobic training, and various forms of resistance training are all the pillars of improving fitness.
I can not recommend anyone of them particularly for any one person. I can only say that the principle behind each program is geared toward challenging your body more so than you would do on your own. Thus the instructor is your motivator.
I have had a few patients come in with back injuries from these programs. So just be careful.
