It's 50ml on the 1st fill up. After that, it's based on how much fuel you actually put in the tank. I used an oral syringe I got from CVS to ensure accuracy. As for the oil, I used Lucas Oil semi-synthetic 2T with low ash from Pep Boys. It was $3.99 for a small bottle, which is enough for 4 fill ups.
