Technically, clutch out first. Once I feel the clutch starting to engage I start feeding in throttle while continuing to release the clutch so that the rpms stay reasonably steady. Basically, it's the same as getting the car rolling with just the clutch, except using a bit of throttle to speed the process up.

Ideally, you want as little clutch slip and as low revs as you can get away with (depending on the situation, that might mean very high revs ), just not to the point that you lug the motor, as that's bad for the bearings/crank.
