It's just usually another "layer" of paint. Most paint these days is 2 stage (base coat/clear coat) the 3 stage paint is usually (base coat/pearl coat/clear coat). There are other varieties and it can get way more complicated than that, but that's just a simplified version of it.
3 stage paint is typically more expensive as the pearl coats are a bit pricey, plus there's added labor of the painter having to do another full layer. Labor difference on something simple (spoiler, side skirt) shouldn't be extreme, but on a whole car it adds up quick!
Oh and $120 to have the Carsmo spoiler sounds like a great price if they do a good job. I was quoted $150-$200 to have same spoiler done in Hot Lava from a reputable shop down here (they do all the work for the local Lexus/Toyota/Scion dealer) and I felt that was reasonable. Hot Lava is also a multi-stage paint that is apparently pricey. :/
