It's dirt and sweat from your hands. You need to get actual leather cleaner and a brush and spend some time working it loose and cleaning it off. For the future, make sure your hands are clean. I keep a pair of gloves in my car in case my hands are dirty or if I'm going on long drives. Two years later, my steering wheel looks new.

In a pinch, some diluted isopropyl works to get the stubborn grime off. But immediately get some conditioner/rejuvenator on it afterwards as the alcohol strips the leather from all of its oils and leaves it dry, which will cause it to crack.

Wash your hand more. Or keep some wet tissues nearby & just wipe ur hand with it.

If you've got super acidy hands, which I do, just get an aftermarket knob and save yourself the trouble. My wheel hasn't started peeling yet, but I'm sure it will someday soon.
